Lenape Valley Foundation Intellectual Disability Supports Coordination is designed to assist individuals and their families in assessing current needs, developing Individual Support Plans, obtaining services and supports, and locating, coordinating and monitoring those supports and services. Eligibility for ID services is based on a diagnosis of Intellectual Disability prior to the age of 22 or a diagnosis of Autism before the age of 22 along with significant limitations in activities of daily living. Information and intakes for ID services in Bucks County are completed by the Bucks County Mental Health/Developmental Programs Department. To determine eligibility, please call 215-444-2800.
